"Blunted affect" ir afekta trulums (the difference between flat and blunted affect is in degree. A person with flat affect has no or nearly no emotional expression).

"Flat affect" precīzāk varētu būt "gandrīz pilnīgs afekta trūkums".
Example sentence:

Psihoanalītiķe Joisa Makdugala ... aizstāvēja ideju, ka afekta (emociju) trūkums ir nevis defekts, bet psihiskas darbības produkts, runājot psihiatru valodā – produktīva simptomātika...

Privātā psihiatrija: afektīvie traucējumi, afektīvās psihozes

emocionāls nomākums

Flat affect and blunted affect are degrees showing to what extent the lack of emotinal expression is present.
Thus, these are medical conditions.

Def.
Flat affect: the absence or near absence of emotional response to a situation that normally elicits emotion. It is observed in schizophrenia and some depressive disorders.
Flat affect: A severe reduction in emotional expressiveness. People with depression and schizophrenia often show flat affect. A person with schizophrenia may not show the signs of normal emotion, perhaps may speak in a monotonous voice, have diminished facial expressions, and appear extremely apathetic. Also known as blunted affect.
