Thanks, I'll write that down, on my list to be discussed on my next medical visit..my doctor just gives me an appointment and then we talk translation over a cup of coffee while the waiting room is full of people, I always find this hilarious and terribly embarassing at the same time.

residuo di arteria polmonare media - not convinced this is "middle pulmonary artery" but I have nothing better to suggest. Can't find any refs to this. Most occurrences mean "middle segment of the pulmonary artery".

Thanks SJLD, that was in fact a very dodgy passage that needed revision, but since you ask the Italian sentence goes: E' presente un residuo di arteria polmonare media che finisce sull'infundibulo destro a fondo cieco.
And no, luckily I "only" have to translate into English this time...

More context We talk child surgery for VSD truncus arteriosus type 1 and a total correction with a Contegra conduit.
Truncus type 1: A residue of the middle pulmonary artery is present that ends in the right infundibulum with no exit (in italian: infundibulo destro a fondo cieco).

This sounds to me like a "hole" that has been made in the aorta during the corrective surgery for this congenital malformation (VSD). It is not possible to be sure without seeing more context. Consider "defect in the aorta"

aortic breach

Non credo si tratti di "apertura aortica" (aortic opening), ossia una normale struttura anatomica presente nel cuore. Qui mi pare si tratti proprio di una lesione, di qualcosa da chiudere.

Il seguente documento descrive una operazione molto simile, solo che il patch non è autologo (ossia non proviene dal paziente stesso), ma di origine animale:
