inglés al español Jurídico/Patentes Contabilidad Annual Returns

I've found these initials "sh. form." in the final page of an annual return I am translating into Spanish. It appears under "Submission No.:" and together with "OR NO.". What could "OR NO." mean? It appears in the receipt of the annual return.
