Certificate of marriage
Hola:
(first) (middle initial) (d) (last)
¿Alguna sugerencia?

Marriage Certificate
Wrong spelling on the name of bride and/or groom
No entries in some of the items in the Certificate of Marriage
Blurred/unreadable entries
Negative result or No record at PSA
Error in the date and place of marriage
Annotation on the annulment/declaration of nullity of marriage
Annotation on the effects of divorce declared in a foreign country
No signatures of the contracting parties (Replacement of PSA copy based on CTC from local)
No signatures/different signatures of issuing officer from concerned LCRO
(first) (middle initial) (d) (last). My thought is that both the mother and father (of whom - (bride or groom?) have only listed their middle initial rather than their full name. In other words, there is a "discrepancy".
Example:
Middle Name | Philippine Statistics Authority
https://psa.gov.ph › ... › Problems and Solutions › Birth Certificate
Middle Name ... middle names like Dela Cruz, Quintos Deles, Villa Roman · Middle initial is entered in the birth certificate instead of the full middle name.

Discussion
(d) = deceased
(d) = difunto
Correct, no “d” above the bride’s father.
