Se me ocurre que es esta la idea, y no me gusta tanto como "me partiría el alma hacerte sufrir", por más que sea una traducción más literal.

: I think this means something like" My heart would go out to you to see you suffer"- slightly different, in that the speaker wouldn't be the one inflicting the pain.

Your context however simply indicates sympathy as in "My heart goes out to you" Perhaps the translation of the original needs to be looked at again.
