In this context the "isolation" refers to removal of cross-over blood flow from the opposite side of the brain. This condition could exist due to congenital abnormality, disease or surgery.
It is a contraindication because the isolated hemisphere would then be left with no circulation at all. That is not considered to be a good thing.
