Über vier Millionen Deutsche leiden an exotischen Krankheiten. Jede einzelne
davon ist so selten, dass sich die Entwicklung eines Medikaments nicht
lohnt. Viele, oft Kinder, müssen deshalb sterben. Wird ein neues Gesetz, das
der Pharmaindustrie mehr Anreize bieten soll, tatsächlich helfen?
Vor einem Jahr war Jonas Achtstätter, 6, aus Aachen noch ein fast normales
Kind. Er konnte sprechen und zählen, sich selbst an- oder ausziehen, ein
Puzzle zusammenlegen, und er liebte es, mit Legosteinen oder
Playmobilfiguren zu spielen.
Seit vergangenem Sommer ist alles anders. Von einem Tag auf den anderen
brauchte Jonas wieder Windeln, statt zu sprechen oder zu spielen schreit er
jetzt oft den ganzen Tag. Blitzartig greift er nach Gläsern, um sie über den
Tisch zu schleudern; Schubladen, die nicht gesichert sind, reißt er hektisch
auf. Binnen eines halben Jahres , sagt seine Mutter, ist Jonas zu einem
schwer geistig behinderten Kind geworden. Nur singen kann er noch wie
vorher. Vor allem Alle Vögel sind schon da .
Jonas leidet an einer seltenen Stoffwechselerkrankung, einer so genannten
Mukopolysaccharidose vom Typ IIIA. Es fehlt ihm ein Enzym, das langkettige
Zuckerverbindungen, die Mukopolysaccharide, in seinen Zellen abbauen kann;
deshalb reichern sich diese Stoffe in Jonas' Nervengewebe an und zerstören
langsam sein Gehirn. Die Krankheit ist erblich; seit einem Gentest ist
gewiss: Jonas' Bruder Nathan, 3, steht das gleiche Schicksal bevor.

Over four million Germans suffer from exotic diseases. Each disease is so rare that the development of new treatments is uneconomical. Many patients, children among them, are doomed to die. Will a new law offering the pharmaceutical industry new incentives actually help?
A year ago six-year old Jonas Achtstatter was a totally normal child. He could talk and count, dress and undress himself, complete a jigsaw puzzle and play with his Lego blocks & toys.
Since last summer all that has changed. From one day to the next Jonas was back in nappies; instead of talking or playing he now often screams the whole day. With lightning speed he grabs glasses & sends them sliding across the table; if drawers aren’t locked, he empties their contents everywhere. “In six months, “ says his mother, “Jonas has become a severely mentally retarded child.” The only thing he can still do like old times is sing, “Kookaburra sits…” mostly.
Jonas suffers from a rare metabolic disorder called MPS type III A. He lacks an enzyme that enables long-chained sugar molecules, mucopolysaccharides, to be broken down in his cells. These then build up in his nerve cells and are slowly destroying his brain. The condition is inherited; a genetic test has confirmed that the same fate awaits his younger brother Nathan.

Cela fut apprécié diversement par les gens, et les Audras par exemple, pourtant gens très pieux, trouvèrent que le jeune abbé allait vite en besogne. Alors Jean-Baptiste, c’était fini? Bien sûr, s’il était allé chez les Frères des Ecoles Chrétiennes à Lyon, ça aurait bien été la même chose, mais enfin là, tout à côté, cela paraissait choquant, et des gens ne se faisaient pas faute de le leur dire, ce qui agaçait le père Audras. Sa femme essayait de le raisonner: “ Que veux-tu, si Jean-Baptiste est heureux, il ne faut rien demander de plus”, mais il n’était pas tout à fait convaincu. Quand arriva le temps de la fenaison, il décida que Jean-Baptiste devait venir aider.

Chapter 10 “Caught in the trap”
Towards the end of March, the first two of Marcellin’s followers donned a special habit intended to separate them from the world. It wasn’t a soutane but rather black trousers with a sort of cloak known as a levite. A small neck ruff was added and a tallish hat.
There was a mixed reaction amongst the locals.The Audras for example, very pious though they were, found the young priest was being just too hasty. Well Jean-Baptiste, is that all there is to it? Of course, if he had gone to the De la Salle Brothers in Lyon, the outcome would have been much the same, but in the end the whole idea seemed shocking, and people were not slow in telling them so, which annoyed old Audras. His wife tried to reason with him: “ Come now, if Jean-Baptiste is happy, we can’t ask for more than that,” but he wasn’t convinced at all. When the time for haymaking arrived, he decided that Jean-Baptiste had to come and help.

La correction chirurgicale du reflux vésico-urétéral représente l’intervention la plus fréquente sur les voies urinaires supérieures de l’enfant. C’est l’opération “phare” de l’urologie pédiatrique. Plusieurs techniques chirurgicales, déjà largement eprouvées et bénéficiant d’un recul important, permettent de supprimer un reflux vésico-urétéral pathologique: ce sont les techniques décrites par Laedbetter et Politano [1] en 1958, par Lich et Grégoir [2] en 1964, par Cohen [3] en 1975. Ces techniques demeurent aujourd’hui la référence. Si le principal critère de choix a pendant longtemps été le taux de succès sur la correction du reflux (et sur ce point la technique de Cohen est la meilleure avec 98% de bons résultats), le concept de traitement mini-invasif a fait son apparition en 1986 avec la cure endoscopique par injection sous muqueuse, méthode ambulatoire totalement indolore mais offrant de moins bons résultats (10 a 15% d’échecs). Du fait de cette concurrence entre chirurgie classique et endoscopie, les urologues pédiatres, fidèles aux techniques classiques, ont multiplié leurs efforts pour simplifier leur geste opératoire, diminuer les douleurs postopératoires, raccourcir le séjour hospitalier, de sorte que depuis 15 ans la prise en charge chirurgicale d’un reflux primitif de l’enfant s’est considérablement allégée.
Notre démarche s’inscrit dans cette ligne et recherche une technique associant les principes de la réimplantation selon Cohen aux avantages de la chirurgie mini-invasive. C’est-à-dire suppression de l’ouverture abdominale et vésicale. Nous présentons ici nos résultats préliminaires.

The surgical correction of vesico-ureteral reflux is the most frequent operation performed on children’s upper urinary tract. It is the leading operation in pediatric urology. Several surgical techniques, effective in resolving pathological vesico-ureteral reflux, have already been proven over the long term: those described by Laedbetter and Politano [1] in 1958, by Lich and Gregoir [2] in 1964, and by Cohen [3] in 1975. These techniques remain standard practice today. While the main criterion of choice in the correction of reflux has for a long time been overall success rates ( with Cohen’s technique incidentally being the best with a 98% good outcome), the concept of minimally invasive treatment became a reality in 1986 with endoscopic submucosal injection, a totally painless outpatient treatment whose results however were less effective (10 to 15% failure rate). Given this convergence between traditional surgery and endoscopy, pediatric urologists trained in classical techniques have striven to simplify their surgical practice, reduce postoperative pain, and shorten patient in-hospital recovery time, with the result that for the past 15 years the surgical management of primary pediatric reflux has been considerably improved.
Our practice is consistent with this trend, and seeks to combine Cohen’s reimplantation principles with the advantages of minimally invasive surgery, leading to a smaller opening in the abdominal wall and none in the bladder. We present here our preliminary results.
